What causes the pain in your back? Your spine has a lot of joints, lyiing behind the spinal joints is a central tube running down from the brain to the sacrum. The membrane contains the cerebo-spinal fluid I which the spinal cord floats and is called the dura mater. All the way down from the neck, pairs of spinal nerves emerge from the dura mater and pass through the apertures between the vertebrae (the intervertebral foramina). Hence, centrally behind the joint lies the dura mater; just to each side lie the nerve roots. These are the two sensitive structures impingement on which causes pain.

When a central backward displacement causes the ligament to bulge out enough to press it agains the dura mater, pain in the centre of the neck, chest or back results, depending at which region of the spine the pressure is exerted. If it passes to one side of the dura mater, the protrusion engages instead against the emergent nerve root. Thus, pain felt in the territory supplied by the nerve root.

Usually when the bulging fragment of disc shifts backwards to one side, it leaves the centre, thus liverating the dura mater. In consequence, as the pain in the upper limb, the front of the trunk or the lower limb comes on, the central pain normally ceases. By contrast, if a disc protrudes sideways or forwards, no sensitive structures lie close to these aspects of the joint, so that even large displacments in those directions cause no symptoms.
